Sample Incident Forms
Happy new year, everyone. I'm curious – do any of you have any sample “incident forms” that you would be able to share? We occasionally have some bumps and bruises occur here on site. Typically nothing that can’t be addressed with a band aid but last week we did have a child need to go get 2 stitches. We plan on better documenting these types of incidents in the future.
