Said: We just have one report for either injuries or accidents that happen here. We use the same form for both general admission guests as well as our Explorer Camp kids. Hopefully it helps with building yours!

Said: We have seperate forms for incidents and injuries. I find the injury to be quite comprehensive but I'm sure the decisions to include the various criteria were based on specific times when those details were needed.Enjoy!
